| District 152 Student Registration Policies |

All parents of students entering a District school for the first time shall present an original, certified birth certificate. Unless registered previously, all kindergarten and first grade students must present an original, certified birth certificate. Students should be registered by their parents. To be eligible to enter school, kindergarten students must be five years old on or before September 1, and grade one students must be six years old by the same date. The State of Illinois requires a Physical Examination of all pupils in public elementary schools immediately prior to or upon their entrance into prekindergarten or kindergarten or grade one and grade five, and all new residents who cannot furnish evidence of a physical examination in the previous school attended. The eligibility of children to enroll in these grades is contingent upon the completion of the Physical Examination. The completed form should be submitted to the school on the day of registration. Proof Of ResidencyParent or Guardian of all students attending District 152 schools shall be required to furnish two (2) proofs of residency at the beginning of each school year or whenever a parent or guardian desires to transfer a student into the District. Proof of residency certification shall be signed by the parent or guardian. For proof of residency, the District will accept only the following items:
All of the items listed above must reflect the address located within the district and shall be certified by the parent or guardian. No student will be permitted to attend school in this district until the proof residency requirements have been satisfied. Textbook Rental and FeesTextbook rental fees are as follows: Pre-Kindergarten $10.00 and $10.00 Technology Fee Kindergarten-6th Grades $20.00 7th and 8th Grade $20.00 + Gymnasium suit $15.00 In addition, all students will pay an instructional/technology fee of $10.00; non-refundable. |
