The 5Essentials survey is a required survey by the Illinois State Board of Education. Our staff and students have been completing the survey, and we encourage our families to participate as well. In the survey, you have the opportunity to share your feedback about your student’s school.
In order for us to receive your feedback, at least 20% of our families must respond. Also, this survey accounts for 5% of a school’s success as measured by the state, so your response will help us improve our overall rating.

If you have not yet taken the survey, please do so by following these steps:
Click “parent survey” and follow the prompts
For the “county” field, type and select Cook
For the "school" field, start typing your child’s school (one of the following)
“Maya Angelou Elem Sch”
“Brooks Middle School-Harvey”
“Bryant Elem School”
“Holmes Elem School-Harvey”
“Riley Preschool”
“Whittier Elem School-Harvey”
Select the appropriate school from the drop-down menu that appears:
Click “Submit Survey” to ensure that your input is recorded.
The survey is open through Friday, March 31.
Please note: Survey responses will never be connected to the identity of you or your child, so your answers are private. School reports will only present grouped information of how parents and students responded as a whole.
